Are you eligible? You need a VA-rated, service-connected disability, or you’re in the VA’s Veteran Readiness & Employment (VR&E) program. You don’t need to be enrolled in VA health care first.
How to register
- Download the form. Download VA Form 10-7959f-1 here.
- Fill it out. Your name, SSN or VA claim number, address, and your service-connected conditions.
- Sign it. Ink signature only. Typed or electronic signatures aren’t accepted.
- Send it in. Email it to [email protected], or fax it to 1-303-331-7803.
- Wait for your letter of benefits. The VA will mail you an authorization letter listing your covered conditions. Once it arrives, send us a copy so we can start billing the VA directly for you.
You don’t have to be registered before you move or travel, but you do need to be registered before we can file anything for you.
